Those who wish to be admitted at VITA Hospice, respectively their parents, relatives, guardians or trustees shall fill in, sign and present the following documents to the Manager:

•    Identity document  /for reference/
•    Contract for hospitalization on a form approved by the Manager;
•    Affidavit for informed consent;
•    Accompanying medical documentation
