What is botulinum toxin?
Botulinum toxin is synthesized and excreted by specific type of bacteria, named Clostridium Botulinum.

Botulinum toxin is synthesized and excreted by specific type of bacteria, named Clostridium Botulinum. In nature they exist only in oxygen-free environment. Several types of botulinum toxin are known - A, B, C, D, E, F and G – and for medical purposes is used mainly type A and very rare type B.
If ingested into digestive system in very big quantities, botulinum toxin (BT) may provoke botulism lesion. Its injection at insignificant quantities directly into the muscles provokes relaxation and smoothing of supernatant skin. Injection into muscles can not provoke botulism lesion.
Since when and in which cases BT is used?
BT is discovered in 1822. Only since 1968 it is used for human medical purposes. Since 1978 it has been applied in ophthalmology (eye diseases) for treatment of strabismus in children, in blepharospasm, in neurology in case of facial hemispasm, torticollis, etc. Since 1992 it is successfully used for treatment of mimic wrinkles of face as well as in one rare but unpleasant status of abundant perspiration, named hyperhydrosis.
What is the mechanism of action of botulinum toxin?
Botulinum toxin is a neurotoxin. It blocks transmission of nervous pulse from neuron to the muscular fiber. Thus the muscle remains in durable status of relaxation i.e. it does not contract and can not wrinkle the skin, which is located over it.
Many interesting but still not yet clarified are the data about the analgesic ability of BT. In patients with migraine, the frequent application of BT leads to disappearance of migraine attacks during its action.
How long does the effect last?
The first changes are reported on hour 72. The maximum effect is after 14 days and the duration of effect is 4-6 months.
The repeated injection may be done within the initial 14 days in necessity of correction and thereafter 2 months the earliest.
The next application of BT is after 6 months and the duration of effect is prolonged to 8-12 months.
Could undesirable effects arise?
Immediately after application, you may feel severity, stretching or headache. This is normal and it will be over soon after resorption of solution. The most serious undesirable effect at application of BT is the eyelid ptosis – fall of eyelid and impossibility to open completely the eye. This status is very rare and is recovered completely within several weeks.
To avoid it, BT is used only by specialists at specialized consulting rooms.
How should I estimate when to come for the next procedure?
Within the time, toxin activity exhausts and the type of wrinkles recovers to a degree close to the initial one. BT advantage is that it provides rest of mimic muscles, due to which the existing lines and furrows do not become deeper and formation of new one is prevented.
Repeated and successive injection of BT may be done at any time when you estimate that the status of your wrinkles is not satisfactory i.e. 1-2 or 3 times in the year as well as simply prior to any special reason for you.
How long (years) may I apply BT?
There is no limitation in the number of therapeutic procedures with BT. Usually after 6 - 8 applications you have well preserved appearance of mimic wrinkles and years may pass prior to undertaking the next steps in the case for beauty and healthy appearance of face.
Is BT therapy suitable for me?
BT is applied successfully in the treatment of mimic wrinkles of the face. These are the wrinkles arising as a result of the permanent activity of facial muscles, when we express some emotions – joy, anger, sorrow, surprise, etc.
In younger age, the skin is elastic and flexible and quickly restores its smooth appearance. At 25 years of age, the first lines and wrinkles appear, and after 30 years of age – the first mimic wrinkles. Most suitable for BT therapy are the patients at the age of 30+ years (30 – 55 years of age). Their skin is with preserved elasticity and does not “fall” after relaxation of muscle.
There is possibility for “preservation” of the facial beauty.
In estimation by a specialist, BT may be applied also in younger patients or at more advanced age, depending on the individual peculiarities, as well as to be combined with other technics – peeling, laser, lifting.
Why should I prefer botulinum toxin therapy?
BÒ is the best non-surgical method for treatment of mimic wrinkles in the region of forehead, around the eyes and between the eyebrows.
No special preparation is necessary prior to or after the manipulation, it is adapted for each patient individually and has reversible effect.
The procedure of BT application lasts about 15 minutes.
No recovery period is necessary during which all people understand that “something happens” with you.
It is injected with super thin needle, due to which no local anesthesia is necessary.
All notice the changes in your appearance without suggesting the reason for it.
The eyebrows are lifted by BT, the eyes open, the face becomes bright and fresh. The look is open and quiet. Expression becomes feminine without any symptom of tension.

For which part of the face BT is recommended?
BT is an aid of first choice in the treatment of mimic wrinkles in the region of the upper half of the face: forehead (at surprising), between the eyebrows, the so called “Lion wrinkle” (at quarrel) and around the eyes – wrinkles type “crow’s feet” (at wide smile). They are formed from the effect of muscles, which accurately and reliable may be defined and treated.
Why it contraindicated for the other parts of face?
In the lower half of the face, the muscles form complex interlacing. The possibilities for clearly anatomic individual peculiarities are bigger here. This may create some difficulties or incorrectness in defining the application places. Therefore in this region some other therapeutic methods are applied, fillers for examples.
I decided to have therapy with BT, how does the procedure pass?
Procedure is carried out at preliminary booked day and hour with Dr. Dinev. The face is cleaned from the make-up, the skin is cleaned on the places where BT shall be applied. The points, where BT shall be applied, are defined and marked. Dr. Dinev specifies the quantity to be injected. The patient is at sitting or reclining position. Photo is taken prior to injection, which shall serve to evaluate the result.
During the injection there will be sense of pricking, stinging or stretching, which fades down immediately after it.
At the end of procedure, you may put cool mask on the face for 10 min.
What shall I do afterwards?
It is recommended to contract and stretch the mimic muscles, making the relevant grimace within 2-3 hours after the procedure. This assists for more uniform and complete distribution po the active substances in the injected muscle.
What shall I NOT do after the procedure?
You should not massage and press the injected places. Do not place active cosmetic products – creams, gels, masques as well as do not undertake cosmetic procedures – peeling, facial massage for at least 72 hours.
